Qualifications: This position does require certification from the State of Ohio as a State Tested Nurse Aide (STNA) or a Licensed Practical Nurse (LPN). One (1) year clinical experience preferred. Job Description: Contributes to all steps of the nursing process to include patient observation, direct patient care, patient teaching within scope, contributing to the planning and evaluation of patient care. Administers medications and treatments and executes regimens authorized by an appropriate individual i.e. physician or advanced practice nurse. Bathes, dresses, and undresses patients, if necessary. Serves and collects food trays and feeds patients requiring help. Transports patients, using wheelchair or assists patients to walk. Takes and records temperature, blood pressure, pulse and respiration rates, and food and fluid intake and output (if necessary), as directed. Cleans, sterilizes, stores, prepares, and issues dressing packs, and other supplies. Gives medication as directed by Physician or Nurse and makes notation of amount and time given. Gives massages, and alcohol rubs, if necessary. Applies hot and cold compresses. May change bed linens, run errands, direct visitors, and answer telephone. May dust and clean patients'' home; be assigned to specific duties by client; assist in providing medical treatment and personal care to patients in private home settings. Contributes to the evaluation of the patients response to care provided through documentation and communication with other members of the health care team. Assists with emergent/crisis situations upon delegation by the RN or physician and prioritizes appropriately. Escalates issues appropriately per the chain of command. Demonstrates critical thinking in the ambulatory care setting. Contributes to the nursing assessment of patients by collecting, reporting, and documenting objective and subjective data in an accurate and timely manner.
